Sec 3 girls - Consent Form for Church Visits

Hey Gals! pls note that I made a typo error on the earlier consent form - the last Sunday should be the 13th Feb, not the 12th.

Dear Sec 3 gals,
please print out consent forms for church visits for the Responsibility in Church badge and submit your forms by 8 Nov to Ms Lim's pigeon hole in school. If you have difficulty printing, collect the form from me at Chapel office.

If there should be a Sunday which you cannot make it, please indicate on the consent form.
